Verify the Portal secrets stored in Mosaic.
Confirm the form in Mosaic and ensure you’ve set the required permissions within Mosaic.
Check if all form Categories have been mapped.
Provide the name of the form as it is displayed in both Mosaic and the portal.
Then please raise a new case online and reference the title of this article,
