This happens when the first element inside a fieldset is hidden by script. This will be fixed in a future version of Mosaic.
The workaround is to add a standard text field as the first element in the fieldeset and uncheck the following options:
Visible in the Basic tab.
Prints with document.
Label visible on print in print options.
