This error occurs when attempting to reopen a step which includes a deactivated form as an optional Activity, even if the form was never actually used.
From Mosaic 22.1.0.0, reopening a step containing deactivated optional templates displays a user-friendly message informing the user that either the form activity is no longer available or, if the optional form is a care package type, that the step contains an optional care package activity.
