This issue affects steps called "Case Records (File Location)" and the related forms called "Case Record details (File Location)", which date back to around 2002 and have been used in many person records.

The problem occurs because the question IDs used in the original form template have changed since the form was created. So, when a user opens the form now, it can’t locate the original questions and shows an "undefined" error.

To fix this, we update the question IDs in the saved answers to match the current ones.

πŸ“Œ Note: Our development team found this same issue at another council; it involved a DATE_ANSWER field using an invalid question ID. Each case needs individual investigation, which is why this KCS record exists only for Hammersmith & Fulham Children's.

The form "Case Records (File Location)" has been used many times there, and the fix is almost always the same, making it easier to resolve when the issue comes up.
