We're pleased to announce that a number of new and updated CoramBAAF health forms are now available within Mosaic as part of the September 2026 release.

Updated forms
The following existing form has been updated:
CoramBAAF Form B Neonatal report
Additional forms have been released by CoramBAAF that are not available within Mosaic. These include forms such as the Young person questionnaire, C-Q Carer questionnaire, and partner forms, which are not intended to be completed directly within Mosaic.
How external forms are recorded and held in Mosaic is being considered as part of future ACG and Mosaic reimagined developments.
