This is a known bug and is on our Development backlog for fixing but with a low priority given the workaround is to remove the double quotes (") or replace with single quotes (').

Steps to replicate:
Go to Person Details > Add Note.
Add a note of type 'Warning' and in the note field included double quotes ".
Save the note and return to the Person Summary screen.
The double quotes " in the Warning note is displayed as ".
