Error when attempting to change CP decision date from step summary screen

Error:

'Expected one result (or null) to be returned by selectOne(), but found: 2'

This error happens if the same question user code appears in more than one form contained in the workstep.

A change will be included in a future Mosaic release to support multiple forms with the same question user code in a Child Protection step, provided only one form has a Child Protection Plan template class.
