Here's how you can meet the key ISN (Information Standard Notice) requirements within Mosaic.

Key Requirements:

  • Identification of disability or impairment:

    You’ll need a local process to identify disabilities or impairment under the Equality Act 2010. Mosaic can support this by providing question prompts throughout your existing workflow and using pre-population from the Current communications needs widget.

  • Recording of disability or impairment:

    You’ll need to have a local process to record disabilities or impairments. Mosaic can support this by providing question prompts record through your workflow and using pre-population from the Current communications needs widget.
    Additionally, disabilities and impairments can be recorded using the conditions and disabilities menu. In line with the ISN recommendations, you may want to align existing conditions and disabilities to the SNOMED CT codes.   • Identification of reasonable adjustments:

    You’ll need a local process to identify reasonable adjustments; Mosaic can support this by using the Current communications needs widget found on the Person summary screen and question prompts throughout your existing workflow.

  • Recording of reasonable adjustments:

    You’ll need a local process to record reasonable adjustments; Mosaic can support this using question prompts throughout your existing workflow and recording directly into the Current communications needs widget.

    📌 Note: From v24.1 the existing widget Current communication needs will be updated to be named Reasonable adjustments and accessible information.

    We’d recommend you update your local system. Align your widget’s category and sub-category codes with the codes listed in the ISN. You’ll find these in Appendix B – List of Reasonable Adjustment SNOMED codes grouped by category (including DCB1605).

    We'd also recommend you create a new Category/subcategory. This should record if a person has a disability or impairment that would be recognised under the Equality Act 2010, along with any relevant details.

  • Flagging of reasonable adjustments (local):

    Mosaics lets you flag reasonable adjustment information by displaying relevant information on the Person summary screen. You can enable the recording of more detailed free text information by creating a 'Note' category for reasonable adjustments information. You might want to use Mosaic’s pre-population functionality within Forms to display all the relevant information in a compact format, as shown in the ISN example 6.4.3.2.

  • Sharing of reasonable adjustments:

    You’ll need a local process to share reasonable adjustments. Mosaic can support this with its existing forms functionality, which can be shared with third party services.

  • Reviewing of Reasonable Adjustments:

    You'll need a local process to review reasonable adjustments. Mosaic can support this by using question prompts throughout your existing workflow.

We'd recommend you update your existing forms to include relevant information.
