Some of our customers have reported finding duplicated forms in some of their worksteps. This has been reported on and off for quite a long time and up to now we have been unable to work out how to replicate this. We were aware that this was likely to relate to incorrect closing of Mosaic windows, so had fed that back to users, but we now have a fuller explanation of how this is possible.

The issue occurs as a result of:
User having a workstep window open but not yet saved.
User returning to the main Mosaic window.
User closing the main Mosaic window using the browser cross in the top right corner of the window or the cross in the window list from the task bar, rather than correctly logging out using the Mosaic 'sign out' button.
User logging back in - for those using single sign on, this log in may not be noticeable to the user, it may seem like they are just re-opening the Mosaic main page.
User starting the same workstep (workstep window 2) from the newly logged in main screen,
User saving workstep window 2 (just with the save icon) and then workstep window 1 (which will have an error, but once that is acknowledged, it will save anyway.
User incorrectly closing the workstep windows with the browser cross and clicking 'leave' when they get a warning message.
Then when the workstep is resumed, it will contain duplicate forms.
In the future we hope to find a way to prevent this from happening.

In the meantime, we would ask customers to ensure their workers are really clear that the browser cross should never be used for closing Mosaic. This is known to cause problems, not just this one, and is not supported use of the system. We recognise this can be difficult and is done by mistake at times, but we are also aware that a lot of users are simply not aware that this is a problem. We request that customers ensure that all their users are clear on this.

We would also recommend that before opening Mosaic, users check that they do not already have any Mosaic windows open, such as workflow steps. If these are open, we suggest that you follow the steps below:
Copy all information that has been added to any forms in that window outside of the system, either manually or by outputting to PDF if this includes all fields.
Log in to Mosaic.
Do not open any other worksteps.
Go back to the workstep window and save and close using the Mosaic buttons.
Acknowledge the error message.
Once that window is closed, open that workstep again, check the content and copy back in anything that is missing and save/close/finish as normal (using only the Mosaic buttons).
